Continuum Care is a healthcare-workflow prototype built around a physician dashboard — "Good morning, Dr. Elena Brooks" — that surfaces a patient panel with condition summaries, recent vitals trends, and pending drafts or replies at a glance.
Clinical dashboards are often dense to the point of being unreadable under time pressure. The challenge was to keep meaningful information density — conditions, vitals, message status — while giving a physician a layout they can scan in seconds between patients.
A sidebar lists the active patient panel; the main view combines summary stat tiles (patients, drafts to publish, awaiting reply, unread) with per-patient cards showing condition tags and a small vitals sparkline. A Physician / Patient view toggle reframes the same data from the patient's side, and a dark-mode toggle is built into the shell.
